Department: Home Care Therapy Under the direction of a physician order, plans and implements specific physical/occupational therapy evaluation and treatment plan programs. The treatment is based on the patient?s physical, developmental and psychosocial needs. Provides advisory service to other team members to provide quality patient care. Performs related duties as assigned.
Minimum Qualifications Education: Graduate of an accredited.degree program in Physical or Occupational Therapy and have passed their national registration exam. Experience: One year prior work experience under the supervision of a qualified Physical or Occupational Therapist. Licensure: Must hold current licensure in the State of Missouri. Certifications: Must have current American Heart Association Heart Saver Provider card or obtain one within the probationary period. Effective 4-1-2010, new coworkers must have a current American Heart Association Healthcare Provider card or obtain one within the probationary period. All Home Care coworkers shall be transitioned to Health Care Provider by June 1,2011. Other: Skills, Knowledge, and Abilities: Must have general knowledge of evaluation and treatment principles for a diverse number and types of diagnoses.Working Conditions, Mental and Physical Requirements: Must drive varying distances to deliver patient care; works in a variety of home and community based environments. Patient care may include heavy lifting and extensive bending and standing. Physical demands are attached plus push, pull, and grip up to 50 lbs., pinch up to 10 lbs., kneel, crouch, and crawl minimally.Equipment Used: May occasionally work with office equipment.
